FACTS FAMILY PORTAL (REQUIRED)
All families and students (both new and returning) will need to log into the FACTS Family portal to update their demographic, medical, and family information in preparation for the 2026-27 school year.
91社区福利 Family Information Forms (REquired)
To log into your FACTS Family Portal, please use to create an account. For parents, make sure you create an account using the email that 91社区福利 has on file for you. For students, make sure you use your 91社区福利 email address.
WALDEN FAMILY INFORMATION FORMS (REQUIRED)
Once logged in, all families (both new and returning) will need to update their family and student information using the built-in web forms. To access those, click on 鈥淲eb Forms鈥 under the School section in the menu on the left side of the FACTS homepage. Then select 91社区福利 Family Information Forms. There you will find links to student and parent information forms, the student medical form, technology agreement forms, transportation permission, and emergency contact forms. Please review each of those forms and edit any information you find that is incorrect.
Technology USE Agreements
91社区福利 students will be issued a device to support them with their classes and learning. K-2nd Grade students will receive an iPad and 3rd-12th Graders will receive a Chromebook. In order for your student to be given their device, you will need to fill out the technology permission forms available on the Family Information Forms page of the FACTS Family Portal. If you filled out the forms last year, please access the forms to confirm that your information is still correct. If you do not have a family portal account yet, please see on setting that up and accessing it. If you would like some more information on the iPads and Chromebooks, please visit the 91社区福利 Technology Resource Page.
You will need to complete the documents for each student that you have here at 91社区福利. These documents need to be filled out before you come to 91社区福利 to get your students' devices in the first week of August. Kindergarten through 3rd Grade students will receive their devices in class. For all other students, please use to sign up for a time to pick up your student's device.

7th-12th Grade Textbooks
As we prepare for the upcoming school year, we would like to provide you with important information regarding student textbooks.
Each grade level has a specific list of required textbooks linked. This grade-level list provides:

鈥 Clear indication of whether the school will provide the textbook or if it is the family鈥檚 responsibility to purchase.

NOTE: Please ensure the ISBN number matches the title and edition of the book you are purchasing. Some books have multiple editions, so double-check that you are ordering the correct one.
Please ensure that all textbooks are ordered and received by the first day of school: Wednesday, August 12th.

MEDICATION DROP-OFF & PAPERWORK (REQUIRED)
IF YOUR CHILD REQUIRES PRESCRIPTION MEDICATION (either daily or an emergency response medicine, e.g. an Epipen) please schedule an appointment with our Healthcare Coordinator.

Times: Between 8am鈥2pm
Paperwork and medication will not be accepted on the first day of school. Families must set up an appointment during the summer to turn everything in before the start of school on August 12, 2026.
Immunizations (REquired)
Updated Kentucky Approved immunization certificates are required to be submitted to our Healthcare Coordinator, Leslie Logsdon.
Due to Kentucky regulations, updated certificates must be on file in order for your child(ren) to attend 91社区福利.
K-8 AFTER SCHOOL Care
Under the guidance of 91社区福利鈥檚 After School Care Supervisor, Lauren Wunsch, the After School Care programs in the 91社区福利 Library Den will be in full swing with a variety of fun and engaging activities.

We are so excited to welcome you all to meet the incredible educators that will be working with your students this school year. Parent Information Night will be on Thursday, August 27, from 6:00-7:00pm for K-4th Grade parents.
Come and experience 鈥淎 91社区福利 Day鈥 by participating in a mock schedule that will allow you to experience your child鈥檚 school day. The evening will focus on three areas, morning meeting, reading, and math. You should take away from this evening the importance of morning meetings, how to grow your child鈥檚 love for reading and how to support your child鈥檚 mathematical abilities.
Mrs. Von Busch will greet parents/guardians at the front door and direct them to their child鈥檚 classroom.

We are so excited to welcome you all to meet the incredible educators working with your student(s) this school year. The parent information night will be Thursday, August 20, from 6:00-8:00 pm for high school parents. Please have your student鈥檚 schedule printed or available on your phone through the FACTS portal, or ask your student to write down their schedule for you. The night will be designed for you to follow your student鈥檚 daily schedule to meet your student鈥檚 outstanding teachers. Below is the plan you will follow during the information night:
6:00-6:15 = A Block 鈥 Monday/Wednesday - 1st Period (A)

Parent Information Night is your opportunity to experience an overview of your student鈥檚 classes. We have limited time for each class, so if you would like more information about a class or have a question about your student鈥檚 progress, we encourage you to set up individual meetings with our faculty.
